Job Title: Patient Access Manager

As a Patient Access Manager at Visiting Nurse and Hospice for VT and NH, you will play a critical role in ensuring seamless patient access to our healthcare services. This position requires a strong leader who can manage a team, prioritize tasks, and maintain high levels of customer satisfaction.

Key Responsibilities:
  • Manage the Patient Access Team in processing new patient referrals, insurance benefit verification, and authorization procurement.
  • Establish and maintain operating policies, processes, and procedures to improve efficiency, quality, and compliance.
  • Monitor results to evaluate compliance with quality and productivity expectations and identify opportunities for improvement.
  • Participate in system-led initiatives and meetings, including system capacity, access, and re-hospitalization.
  • Establish customer service standards of excellence and ensure compliance.
Requirements:
  • Bachelor's degree in nursing from an accredited college or university preferred.
  • Registered Nurse or Licensed Practical Nurse from an accredited college or university with experience in coding, scheduling, and referral management preferred.
  • Three (3) years' experience in a direct supervisory role required.
  • Experience in a healthcare setting with working knowledge of medical terminology, electronic medical records, and billing systems/software required.
  • CHAM - Certified Healthcare Access Manager desired, or certification within 1 year of hire required.
