Registered Dietician

We are seeking a highly skilled Registered Dietician to join our team at the Hampton Roads Community Health Center, Inc. The ideal candidate will have a strong background in nutrition and experience working in a clinical setting.

The Registered Dietician will be responsible for collaborating with our interdisciplinary healthcare team to improve the health and physical conditions of our patients' nutritional needs. This includes serving as a subject matter expert in diabetes management and providing counseling, nutritional assessments, prescriptions, and evaluation of nutrition-related lab parameters to improve results and maximize outcomes.

• Analyze patients' health status, goals, and dietary restrictions, tracking dietary intake and progress, and educating patients on how to maintain a balanced diet and optimum health.
• Create or gather educational materials/handouts used to educate patients and provide counseling and nutritional assessments.
• Conduct comprehensive nutrition assessments using the Nutrition Care process (NCP) model, including nutrition-focused physical exams (NFPE), and participate in review of class and outcomes every 6 months.
• Develop and implement individualized therapeutic plans for specialized needs and monitor nutritional status, laboratory values, medications, dialysis kinetics, adherence, and response to diet and/or other nutrition therapy.
• Document all clinical actions in the electronic medical record and review the chart for completeness, test results, and billing.

• Bachelor's degree in Nutrition or related field and at least three (3) years of dietetic experience in a clinical setting or any equivalent combination of education and experience.
• Must have completed a state-approved certification program for a Registered Dietitian or Certified Diabetic Educator.
• Must be certified in Basic Life Support techniques.
