As a Hospice Nurse, you will play a vital role in supporting and educating patients and their families as they navigate the end-of-life journey. You will draw on your experience, skills, and compassion to provide exceptional care and support.
Key Responsibilities:
- Perform referral evaluations and admission assessments of patients and families seeking Hospice services
- Conduct physical, psychosocial, and spiritual assessments to identify patient needs
- Communicate patient and family expectations, goals, and learning needs to the healthcare team
- Provide Case Management functions to assigned patients
- Deliver care that meets professional standards and promotes patient comfort and dignity

Requirements:
- Degree from an accredited school of nursing (BSN preferred)
- Current WI RN license
- 1-2 years of Hospice, Oncology, or related experience
- Hospice certification required within 18 months of employment
- Current/valid WI driver's license and reliable/insured vehicle
Work Schedule: Part-time, 16 hours/week, 2nd shift (3pm-11:30pm), including weekend and holiday rotation.
