About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Assessment Visits Professional. As a key member of our home-based care team, you will provide high-quality clinical services to patients in their homes, ensuring their comfort, safety, and well-being.

Key Responsibilities
  • Conduct thorough clinical assessments of patients in their homes, identifying their unique needs and developing personalized care plans.
  • Provide and coordinate nursing and personal care services to patients, as prescribed by physicians or authorized by state/program regulations.
  • Collaborate with other healthcare professionals, including physicians, case managers, and community resources, to ensure seamless patient care.
  • Ensure patient eligibility and medical necessity for services, in accordance with payer requirements and agency policies.
  • Develop and revise individualized plans of care and service plans, as required by federal and state program regulations.
Requirements
  • Current RN licensure in the state of practice.
  • Minimum one year of experience as an RN, preferably in home health or a related field.
  • Current CPR certification.
  • Access to a dependable vehicle or public transportation.
