Job Summary

We are seeking a skilled LPN Nurse to join our team at a long term care, memory care, short-term rehabilitation and senior independent living facility. This is a full-time, onsite position that requires a strong commitment to providing exceptional patient care.

Key Responsibilities
  • Administer medications and treatments in accordance with established standards and protocols.
  • Provide safe and efficient personal care to residents, utilizing recommended safety devices and adhering to infection control standards.
  • Respond to resident calls and requests for assistance, while maintaining a high level of professionalism and respect.
  • Document all medications and treatments administered, as well as care provided, in a timely and accurate manner.
  • Participate in resident care planning, updating care plans and aide assignments as needed.
  • Assess residents, document findings, and report to the physician, while transcribing physician orders accurately.
  • Communicate resident changes in condition to resident sponsors, while maintaining confidentiality and respect.
  • Dispose of discontinued medications, maintaining accurate records and adhering to facility policies.
  • Maintain accurate controlled substance numbers during shifts, while adhering to facility protocols.
  • Complete required documentation, including nursing notes, transfer forms, and other records.
  • Assist with the operation of the unit, including the monthly changeover of records.
  • Complete and distribute nurse aide assignments, while ensuring accurate and timely communication.
  • Provide report to oncoming shifts, while maintaining a high level of professionalism and respect.
  • Schedule and assist with physician, optometrist, podiatrist, and dentist appointments, while maintaining accurate records.
  • Administer and record employee health testing results, while maintaining confidentiality and respect.
Requirements
  • Indiana Professional License or License from an NLC participating state required at time of submission.
  • Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records.
  • Hands On - American Heart Association CPR and BLS Certification required.
  • Dementia 6 hour certification is required prior to start.
  • LTC experience required.
Preferred Qualifications
  • EMR, Matrix Care experience preferred.
  • Experience with Memory Care.
  • Experience with short-term rehab.
Physical Demands
  • Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state and local standards.
  • Sedentary work that involves sitting or remaining stationary most of the time with occasional need to move around the office to attend meetings, etc.
  • Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor.
