About the Role

We are seeking a skilled Registered Nurse (RN) to join our Home Health team as a PRN Registered Nurse (RN) in a Flexible Schedule capacity. This role offers the opportunity to provide direct patient care within homes, making a meaningful difference in the lives of our patients.

Key Responsibilities
  • Conduct comprehensive assessments and complete timely OASIS documentation
  • Draft and implement patient care plans, reconcile medications, and ensure accurate orders
  • Prepare and maintain accurate, timely documentation
  • Coordinate care with patients and provide reports to Case Managers
  • Provide health education to patients and families as needed
Requirements
  • Active RN license in the state of practice or eligibility to take NCLEX
  • Strong communication and interpersonal skills
