Job Summary

The Occupational Therapist position at First State Home Health & Hospice Care is responsible for developing and implementing occupational therapy programs and providing occupational therapy treatments to patients in their homes. The successful candidate will facilitate increased independence and functioning, and provide supervision of other Occupational Therapy Assistants and caregivers.

Key Responsibilities
  • Attend to patient care at the patient's home, while also providing direction to Occupational Therapy Assistants and other caregivers.
  • Responsible for the effective and efficient delivery of rehab services; provision of and adherence to an established plan of care, department and patient(s)' schedules, and timely documentation of services.
  • Work with other members of the rehabilitation team to develop programs and activities consistent with the needs and capabilities of each patient.
  • Instruct patients, families and other therapists/caregivers in the skills and techniques of the occupational therapy treatment program.
  • Maintain appropriate and timely documentation for all patients treated.
  • Clean, maintain, identify, and report the need for repair of equipment, as necessary.
  • Puts Patients First: Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
  • Develops and implements patient's individualized treatment plan.
  • Provides individualized occupational therapy interventions including application of therapeutic techniques in such areas as, but not limited to: cognitive skills, sitting and standing tolerance, muscle strength, balance, coordination, activities of daily living, endurance, joint protection, mobility, work simplification, perceptual abilities, orientation, sensory awareness, physical agent modalities.
  • Performs other related duties as required.
Requirements
  • Graduate of an accredited Occupational Therapy Program.
  • Licensed and/or eligible for licensure as required in the State of Michigan.
  • Valid driver's license, and reliable transportation.
  • Familiarity with the DeVero healthcare software system and OASIS is a plus.
  • Ability to communicate clearly, both verbally and in writing.
  • Ability to reliably attend patient appointments and staff meetings.
