Job Summary:

  • Collaborate with the interdisciplinary team to provide optimal patient experience and meet patient needs.
  • Plan, prepare, and carry out individually designed programs of occupational therapy treatment to maintain, improve, or restore function to all ADL and IADL.

Responsibilities:

  • Evaluate patient data to identify functional impairments and manage caseload by scheduling appropriate therapy revisits and reassessments.
  • Confer with patients, family, caregivers, physician, and multi-disciplinary team members to plan, implement, and assess intervention programs.
  • Administer ADL/IADL retraining, therapeutic exercises, therapeutic activity, range of motion treatment, home safety training, and cognitive retraining to improve function and quality of life.
  • Instruct patients, family, and caregivers in treatment procedures to increase carry-through with the plan of care and improve patient outcomes.
  • Recommend equipment and home modifications to improve patients' function in the home environment and caregivers' ability to help patients.
  • Perform medicine reconciliation and monitor patient vital signs and physiological response to treatment sessions.
  • Discharge patients from occupational therapy when goals or projected outcomes have been attained or patient is no longer homebound.
  • Assign patients to occupational therapy assistants as appropriate and provide supervision according to the OT practice Act and PCAH policies.

Credentials:

  • Basic Cardiac Life Support (Required)
  • Car Insurance (Required)
  • Driver's License (Required)
  • Occupational Therapist (Required)

Education or Equivalent Experience:

  • Bachelor's degree in Occupational Therapy from an ACOTE accredited school of Occupational Therapy (Required)
  • Master's degree or Doctorate degree (Preferred)
