The Clinical Healthcare Coordinator role at CAI is an exciting opportunity to provide community-based care coordination and management for clients in Delaware. This position requires assessing, planning, and implementing care for members with complex healthcare needs while collaborating with healthcare providers.
About the RoleWe are seeking a motivated and organized Clinical Healthcare Coordinator to join our team. As a key member of our care coordination team, you will be responsible for assessing, planning, and implementing care for members with complex healthcare needs. You will work closely with healthcare providers to ensure that members receive the best possible care.
This is a full-time position that requires travel to members' homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments. The ideal candidate will have excellent communication and organizational skills, as well as experience working with complex healthcare populations.
Key Responsibilities- Assess, plan, and implement care for members with complex healthcare needs
- Collaborate with healthcare providers to ensure that members receive the best possible care
- Travel to members' homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments
- Document all case management services and interventions in the electronic health record
To be successful in this role, you will need:
- A current Registered Nurse (RN) license in the state of Delaware
- At least 2 years of experience in case management and discharge planning, including experience discharging members from a facility setting
- Experience completing assessments, developing service plans, and coordinating care across the continuum of services
- Excellent communication and organizational skills
- A valid driver's license and reliable transportation
The salary for this position is $65,000 per year, plus benefits. We offer a competitive salary and a comprehensive benefits package, including medical, dental, and vision insurance, as well as a 401(k) retirement plan.
