* Conduct holistic assessments and develop care plans in partnership with members and their caregivers
* Coordinate care with community resources and healthcare providers to ensure effective transitions of care
* Provide telephonic case management support to high-risk members, focusing on those enrolled in the Dual Eligible Special Needs Plan
**Required Qualifications and Training:**
* Unrestricted, compact RN license with willingness to obtain non-compact state licenses if needed
* Prior case management experience with the D-SNP population
* Experience working with dual eligibles in some capacity
* Bilingual in English and Spanish is a plus
* Health insurance experience, particularly in Medicare Advantage, is a plus
