Job Summary

We are seeking a highly skilled and compassionate Activities Coordinator/Social Worker to join our team at WVU Medicine. As a key member of our healthcare team, you will be responsible for designing and implementing patient-centered activities programs that promote physical, emotional, and social well-being.

Key Responsibilities
  • Develop and implement individualized activity plans that meet the unique needs and interests of patients.
  • Collaborate with healthcare professionals, family members, and other stakeholders to ensure seamless care coordination.
  • Provide administrative support to the ECF Department, including data collection and analysis, supply ordering, and budget management.
  • Assess patient needs and preferences to identify opportunities for meaningful engagement and participation.
  • Communicate effectively with patients, families, and healthcare teams to ensure patient-centered care.
  • Develop and maintain relationships with community resources and organizations to enhance patient services.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Bachelor's degree in a related field, such as social work, recreation therapy, or a related field.
  • Minimum one year of experience in a skilled care setting, preferably in a patient activities program.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
  • Basic computer skills and proficiency in electronic medical records.
Working Environment
