Key Responsibilities:- Provide in-home primary care to our most frail and at-risk elderly patients
- Develop and implement personalized care plans to meet the unique needs of each patient
- Collaborate with patients and their families to ensure seamless care coordination
- Work autonomously as part of a cohesive team to achieve exceptional patient outcomes
- National Board Certification
- Licensed by the Commonwealth of Massachusetts board of registration in Nursing
- Graduate of an Accredited Nurse Practitioner Program, Masters level preferred
- Maintains BLS certification, professional certification as appropriate
