Life Care Center of Kona is currently seeking a skilled and dedicated Physical Therapist to join our interdisciplinary team. As a Senior Rehabilitation Therapist, you will play a vital role in providing exceptional patient care and contributing to the success of our facility.

About the Role

We are offering a competitive salary range of $85,000 - $110,000 per year, depending on experience. This full-time position comes with a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.

Job Description

In this role, you will be responsible for developing and implementing individualized rehabilitation plans for patients. You will work closely with our interdisciplinary team to ensure seamless communication and coordination of care. Your expertise will be essential in helping patients achieve their goals and regain independence.

Your key responsibilities will include:

  • Conducting thorough assessments and evaluations to determine patient needs
  • Developing and implementing personalized treatment plans tailored to each patient's goals and abilities
  • Collaborating with other healthcare professionals to ensure comprehensive care
  • Providing education and training to patients and their families on proper techniques and strategies for recovery
  • Maintaining accurate and up-to-date records of patient progress and outcomes
Requirements

To be successful in this role, you must have:

  • A Master's degree in Physical Therapy (MSPT) or Doctorate in Physical Therapy (DPT)
  • Current licensure as a Physical Therapist in the State of Hawaii
  • At least one year of experience in post-acute care or a related setting
  • Strong communication and interpersonal skills
  • Able to work independently and as part of a team
  • Familiarity with Microsoft Office and electronic medical records systems
