Job OverviewPosition Title: Clinical Case Coordinator

Position Summary:

The Center for Alternative is in search of a dedicated and empathetic Clinical Case Coordinator to enhance our Pretrial Services Case Management initiative. In this role, you will deliver oversight and community-oriented services to a designated group of pretrial participants, collaborating closely with them to facilitate their successful adherence to mandated supervision requirements and court obligations.

Work Schedule: This is a full-time position with hours from Tuesday to Saturday, either 9 AM to 5 PM or 10 AM to 6 PM.

Compensation: The annual salary for this position is $67,500.

Workplace Flexibility: This role offers flexibility, allowing for partial remote work when appropriate, such as drafting notes from home after morning engagements.

Key Responsibilities:

  • Deliver oversight and community-based services to a caseload of participants.
  • Conduct orientation meetings within 24-48 hours of a participant's release to discuss supervision conditions, reporting schedules, and communication methods as outlined in their supervision plan.
  • Collaborate with the court and pretrial staff to ensure participants meet their supervision and court obligations, reviewing legal documentation and assisting participants in understanding their behavior patterns to foster positive change.
  • Perform ongoing assessments to identify participants' psychosocial needs, including mental health and substance use challenges, and connect them with suitable treatment options.
  • Facilitate effective outreach and case management activities to achieve program objectives.
  • Employ problem-solving techniques and motivational interviewing to engage participants in their change processes.
  • Establish and maintain collaborative relationships with community service providers, including housing, health care, and educational resources.
  • Address complex issues and adhere to community-based monitoring protocols, particularly for high-risk participants, while ensuring accurate and confidential documentation of all case management activities.

Qualifications:

  • Master's degree is required.
  • A minimum of 2-3 years of experience in social services, particularly with individuals facing substance use issues and involvement in the criminal justice system.
  • Familiarity with community treatment options and support services.
  • Understanding of diverse family systems and cultural backgrounds.
  • Highly organized and self-driven.
