Physical Therapist

We are seeking a skilled Physical Therapist (PT) to join our team at Aequor Technologies. As a Physical Therapist (PT), you will be responsible for assessing the physical abilities and needs of patients in a skilled nursing facility, developing individualized therapy plans, and providing physical therapy treatments to improve mobility, strength, and overall functional ability.
Responsibilities
* Assess patients' physical abilities and needs in a skilled nursing facility
* Develop individualized therapy plans to improve mobility, strength, and functional ability
* Provide physical therapy treatments to patients, including exercises and activities to promote recovery and independence
* Collaborate with an interdisciplinary team, including physicians, nurses, and occupational therapists, to coordinate care and ensure optimal patient outcomes
* Educate patients and their families on therapeutic exercises, home programs, and strategies to maintain progress post-discharge
Requirements
* Current Physical Therapist (PT) license in good standing
* Minimum of 2 years of experience in a skilled nursing facility (SNF) or similar setting
* Strong knowledge of rehabilitation techniques, therapy equipment, and treatment planning in a long-term care environment
* Excellent communication and interpersonal skills
* Ability to work collaboratively with a multidisciplinary team and independently
