Nurse Practitioner

As a Registered Nurse with IntelyCare, you will be responsible for providing high-quality patient care in post-acute care facilities. This includes:

• Creating and implementing individualized care plans in collaboration with medical staff and residents.
• Delivering treatments and administering medications to residents, while ensuring their safety and well-being.
• Reviewing care options and outcomes with physicians and other staff, and making recommendations for improvement.
• Educating residents and family members about best options for care, and providing support and guidance as needed.

To be successful in this role, you will need:

• A current Pennsylvania state license to practice as a Registered Nurse (RN).
• Completion of an approved nursing program.
• At least 4 months experience in either post-acute care or in-patient hospital settings.
• Strong communication and organizational skills, with the ability to work independently and as part of a team.
