About the Role:

The Registered Nurse (RN) Case Manager II is a critical member of our multidisciplinary team, responsible for facilitating seamless patient transitions from acute care settings to appropriate levels of care. This role requires strong communication and collaboration skills to ensure effective coordination among healthcare providers.



Key Responsibilities:
  • Patient Transition Planning: Develop and implement individualized transition plans to ensure smooth handovers and continuity of care.
  • Collaboration with Healthcare Providers: Work closely with physicians, nurses, and other healthcare professionals to ensure timely and effective communication regarding patient needs and progress.
  • Care Coordination: Facilitate the transfer of patients to the next level of care, ensuring that all necessary information and resources are available to support optimal outcomes.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care and safety, identifying areas for improvement and implementing evidence-based solutions.


Requirements:
  • Education: Graduate from a CCNE or NLN accredited nursing program; Bachelor's degree in nursing preferred.
  • Experience: Four years related experience and/or training; or equivalent combination of education and experience preferred.
  • Licensure/Certification: Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.
