**Job Description:**
As a Family Support Navigator at Stanford Sierra Youth & Families, you will play a critical role in supporting the well-being and empowerment of youth and families in our community. Your expertise in community resource linkage, case management, and advocacy will enable you to provide high-quality services that promote healthy relationships, resilience, and self-sufficiency.
You will work collaboratively with internal programs, departments, and staff to coordinate care and provide support as needed. Additionally, you will develop and maintain positive relationships with community partners, families, and providers to ensure effective referral and linkage to agency continuum services.
Key Responsibilities:
- Engage and support engagement at various community locations but not limited to resource centers, homeless shelters, schools and other locations to increase community awareness and linkage of youth & families to the agency Medi-Cal and/or Medi-Cal covered services.
- Provides follow-up support & contact to ensure youth & families are appropriately linked and connected to the agency continuum of services and/or Medi-Cal services.
Requirements:
- Bachelor's degree in social service, psychology, juvenile justice, sociology, child development or health/human services related field AND 2 year experience providing community engagement, community resources/linkage, or direct service support to youth & families.
- Possess and demonstrate specific cultural competence to work with and outreach to culturally diverse populations and communities.
