POSITION SUMMARY
The primary responsibility of the facility-based Advanced Practice Provider (APP) is to deliver high-quality, provider-level direct patient care services within the Post-Acute Care environment.
KEY RESPONSIBILITIES
- Perform clinical interventions and episodic care visits, along with necessary follow-up evaluations.
- Manage ongoing assessment and treatment of chronic and co-existing health conditions, as clinically indicated.
- Assess and address changes in residents' health status.
- Review and manage medications, orders, and diagnostic tests, responding appropriately to any clinical irregularities for all patients under their supervision, as permitted by collaborating physicians.
- Collaborate with physicians to ensure comprehensive medical management.
- Conduct Annual Wellness Visits for all long-term care residents and perform Annual History and Physicals as requested.
- Assist with facility-wide health concerns, where collaborative opportunities exist.
- Coordinate care with other healthcare professionals, including Therapists, Specialists, and Social Workers.
- Maintain open communication with families and participate in family conferences when feasible.
- Perform additional duties as assigned.
- Active nurse practitioner or physician assistant license in the state of practice & current board certification in your specialty.
- Master's Degree in Nursing or Physician Assistant Studies.
- Preferred strong leadership, interpersonal, and problem-solving capabilities.
- Excellent written and verbal communication skills are essential.
- Demonstrated punctuality, organizational skills, and ability to work effectively within a team.
