Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Elmhurst Hospital Center in Queens. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our staff to deliver exceptional outcomes.

Key Responsibilities
  • Provide comprehensive medical and nursing care to patients and families, including preventive and supportive health services.
  • Manage health problems and coordinate healthcare for various patient populations, identifying and diagnosing illnesses and physical conditions.
  • Develop and maintain partnerships with community-based organizations and make referrals to consulting physicians and/or other healthcare professionals as needed.
  • Evaluate and interpret patient data to plan and implement appropriate nursing interventions.
  • Consult with physicians and other healthcare practitioners to make prospective decisions about treatment that will best support improved outcomes and patient care.
  • Formulate differential diagnoses and plans of care based on medical history, physical exams, nursing assessments, and other pertinent information.
  • Develop, implement, and evaluate patient plans of care, determining effectiveness and modifying as needed to achieve medical and health goals.
  • Order, conduct, and interpret diagnostic and laboratory tests, identifying and prescribing appropriate pharmacological and non-pharmacological interventions and therapies.
  • Manage patient session schedules, ensuring availability and continuity of care, and utilizing resources effectively.
  • Provide education, guidance, and direction to nursing staff and other healthcare team members to ensure quality and safe medical and nursing care.
  • Provide education and counseling to patients and families regarding health status, treatment, and progress, medication management, disease prevention, and health promotion.
Requirements
  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
  • A current certificate as a Nurse Practitioner issued by the NYSED.
  • A valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
