Job Summary:We are looking for a registered nurse with a strong passion for delivering exceptional patient care in a home health setting. As a per diem nurse, you will have the opportunity to work with a variety of clients, each with unique needs and circumstances.
Responsibilities:- Provide skilled nursing care to clients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each client.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Communicate effectively with clients, families, and caregivers to ensure understanding and compliance with care plans.
- Document all interactions, interventions, and outcomes in a timely and accurate manner.
- Current RN licensure in Colorado.
- At least 1 year of experience in home health or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work a variety of shifts, including weekends and holidays.
