Riverside Health System is seeking a skilled MDS Coordinator RN to join our team at our Riverside Sanders location in Gloucester, VA.

About the Role
As an MDS Coordinator RN, you will play a critical role in ensuring the accurate and timely completion of the Resident Assessment Instrument (RAI) process. You will work closely with the Administrator and other members of the care team to deliver high-quality care, improve resident outcomes, and optimize reimbursement and quality assurance.

Key Responsibilities

  • Report the status of Minimum Data Set (MDS) assessments to the Administrator on a weekly basis, ensuring timely scheduling and completion of assessments in accordance with regulatory requirements.
  • Utilize auditing tools to manage RUG, ADL score, 5 Star, QRP, and QM trends, providing insights to inform care decisions.
  • Communicate with the Accounts Receivable analyst regarding PPS assessments ready to be billed, ensuring timely completion and submission.
  • Participate in meetings and training events provided by the Quality/Reimbursement team, staying up-to-date on best practices and industry developments.
  • Provide in-house training for direct care staff to ensure accurate documentation of provision of care, promoting a culture of excellence.
  • Participate in monthly facility Quality Assurance (QA) meetings, focusing on QM/5 Star/QRP, and serve as a resource to the QA team in developing QAPI plans related to QM/5 Star reports.
  • Collaborate with nursing administration, providers, and rehab to determine the best course of action regarding quality care and provision of care, implementing plans of care as needed.
Requirements

Experience
  • 2 years of experience in long-term care or skilled nursing (preferred)
Licenses and Certifications
  • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) or licensed in a compact state (required)
  • BLS/CPR/AED - American Heart Association within 30 days (required)
  • Certified MDS Assessment Coordinator (CMAC) (preferred)
