We are seeking a compassionate and skilled Caregiver for Seniors to join our team at InnovAge. As a Home Care Assistant, you will provide personalized care and support to seniors in their own homes, enabling them to maintain their independence and dignity.

In this role, you will be responsible for assisting with activities of daily living, such as bathing, dressing, grooming, and toileting. You will also assist with restorative and supportive care, including ambulation, positioning, and skin care. Your goal will be to promote the overall well-being and health of our participants.

The ideal candidate will possess excellent communication skills, empathy, and patience. You will work closely with our multidisciplinary teams to develop personalized care plans and ensure seamless transitions between services.

Responsibilities:

  • Assist participants with activities of daily living, such as bathing, dressing, and grooming
  • Provide restorative and supportive care, including ambulation, positioning, and skin care
  • Monitor participants' vital signs, weights, and skin condition
  • Assist with meals and feeding, as needed
  • Report any changes or concerns to the clinical supervisor

Requirements:

  • Current and valid state issued Certified Nursing Assistant (CNA) license
  • Current CPR certification or the ability to obtain CPR certification
  • Require personal transportation and a current state issued driver's license, good driving record, and auto insurance
  • A minimum of one year experience working with frail or elderly individuals
