Job Summary

IBUKUN Comprehensive Community Serv is seeking a highly skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive case management services to children and families in need.

Key Responsibilities
  • Participate in Intake Team Decision-Making: Collaborate with the intake team to make informed decisions regarding case placement and management.
  • Evaluate Goals for Placement: Work with D.C.F.S. workers to establish and evaluate goals for placement in cooperation with the child and their family.
  • Manage Caseload: Supervise and manage a caseload of children in placement at the group home or Transitional Living Program and ILO.
  • Participate in Administrative Case Reviews: Participate in Administrative Case Reviews (ACRs), attend court hearings, and maintain liaison with D.C.F.S. staff.
  • Screen Natural/Foster Parents: Assist in screening natural/foster parents to ensure they meet visitation requirements and minimize exposure to potential abusive experiences.
  • Recommend Termination: Recommend termination of children and maintain required records.
  • Perform Administrative Duties: Perform other administrative duties as assigned by the Clinical Program Director, including supervision of Child Welfare Workers.
  • Coordinate with Community Agencies: Coordinate with community agencies to carry out treatment plans.
  • Provide Direct Services: Provide direct services as indicated in the treatment plan.
  • Support and Counseling: Provide support, counseling, and education to clients to assist them in achieving their goals.
  • Assist with Benefits and Services: Assist clients in accessing available benefits and services.
  • Motivational Interviewing: Utilize motivational interviewing and provide appropriate mental health interventions.
  • Maintain Professional Standards: Maintain all applicable professional, legal, and ethical standards, including confidentiality, dual relations, and informed consent.
  • Clinical Documentation: Provide clinical documentation, reports, and records to meet State, Federal, Regional, and departmental standards and requirements.
Qualifications
  • Bachelor's Degree: A Bachelor's degree in Social Work or a related field such as Psychology, Sociology, or Criminal Justice.
  • Experience: At least two years of experience in Child Welfare or at least field work experience in this field.
