Role Overview:

The Physical Therapist will develop and implement individualized physical therapy programs, focusing on assessment, treatment, planning, education, execution, and communication to facilitate patient progress towards functional goals. The role also involves building positive relationships with fellow healthcare professionals and support staff.

Key Responsibilities:

  • Design, implement, and monitor patient treatment plans.
  • Communicate effectively with patients, families, referring physicians, and team members.
  • Provide timely documentation in accordance with facility and departmental standards, as well as insurance requirements.
  • Enhance clinical and professional expertise through ongoing education and professional connections.
  • Participate in departmental initiatives and improvements.
  • Alert the Director of Rehab and interdisciplinary team regarding any changes in patient condition or needs.
  • Supervise Physical Therapy Assistants to ensure optimal patient care and achievement of goals.

Qualifications:

  • Current Florida License as a Physical Therapist (PT)
  • Valid CPR certification
  • Preferred: Minimum of one (1) year experience in a Skilled Nursing Facility.
