Job Responsibilities:

The Licensed Practical Nurse (LPN/LVN) will be tasked with delivering and documenting skilled nursing care under the supervision of a Registered Nurse, following the established care plan and physician directives for each patient while maintaining confidentiality and professional boundaries.

Key Responsibilities Include:
  • Delivering and coordinating quality patient care as per physician orders.
  • Monitoring and assessing patient conditions, reporting any changes to the supervising nurse or physician.
  • Documenting all nursing actions, assessments, and care coordination promptly.
  • Participating in the development and updating of nursing care plans.
  • Taking appropriate nursing actions based on assessments to achieve desired outcomes.
  • Identifying changes in patient needs and implementing necessary interventions.
  • Adhering to all policies, regulations, and standards of practice.
  • Utilizing infection control measures to safeguard both staff and patients.
  • Educating patients and families on disease processes, self-care techniques, and preventive strategies.
  • Maintaining professional competencies through ongoing education and training.
Qualifications:
  • Graduate from an accredited nursing program.
  • Current, unrestricted state license as a Licensed Nurse.
  • Valid CPR certification.
  • Proven proficiency in clinical assessments and nursing documentation.
Preferred Experience:
  • At least six months of recent nursing experience in a clinical setting.
  • Experience in home healthcare.
Skills and Abilities:
  • Strong attention to detail.
  • Effective time management skills.
  • Problem-solving and conflict resolution abilities.
  • Excellent organizational and communication skills.
Physical Requirements:
  • Ability to communicate effectively in English.
  • Capacity to travel as needed.
  • Ability to lift up to 50 pounds.
  • Physical stamina for prolonged walking, standing, and other movements.
  • Capability to respond appropriately to emergency situations.
Work Environment:
  • Ability to work in various environments, potentially exposed to allergens and other conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
