About the Role:

This is an exciting opportunity to join our Home-based Medical Care team in Queens, NY. As a Nurse Practitioner or Physician Assistant, you will be responsible for providing high-quality medical care to complex, chronic patients in their homes. You will work closely with primary care physicians and other healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of each patient.

Responsibilities:
  • Perform initial comprehensive assessments and bi-annual follow-up assessments as needed
  • Develop and implement comprehensive care plans in collaboration with primary care physicians and other healthcare professionals
  • Communicate effectively with patients, families, and caregivers to ensure seamless care coordination
  • Participate in interdisciplinary team meetings to share knowledge and best practices
  • Prescribe appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissions
Requirements:
  • Completion of Nurse Practitioner or Physician Assistant accredited program
  • Current state RN license (if Nurse Practitioner)
  • Current NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
