Job Summary

The Telehealth LPN is a vital member of our healthcare team, providing high-quality patient care and support in a variety of settings. As a Telehealth LPN, you will work under the direction of a Registered Nurse (RN) or Provider to deliver safe and effective care to our patients.

Key Responsibilities
  • Perform patient assessments and develop individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and their families on various health topics, including medication management, disease prevention, and healthy lifestyle choices.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Administer medications and treatments as prescribed by the RN or Provider.
  • Monitor patient vital signs and report any changes or concerns to the RN or Provider.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate of an accredited school of practical nursing.
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of employment.
  • Basic Life Support (BLS) certification.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
Preferred Qualifications
  • Experience working in a telehealth or home healthcare setting.
  • Knowledge of electronic medical records (EMRs) and other healthcare software.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
