We are seeking a talented Patient Care Physical Therapist to join our team at CommonSpirit Health at Home. As a Patient Care Physical Therapist, you will have the opportunity to work in a dynamic environment, providing direct patient care in the comfort of their own homes.

Your responsibilities will include developing, implementing, and supervising the home care plan for the physical rehabilitation of patients under the direction of the attending physician and clinical branch leadership. You will perform initial evaluations, establish care plans, and revise them based on changes in the patient and the environment.

This role requires a Bachelor's degree from an accredited school approved by the American Physical Therapy Association and current licensure to practice in the state serviced. Additionally, you should possess current CPR Certification and have intuitive knowledge of technology and be comfortable with smartphones, tablets, virtual reality, interactive software, and artificial intelligence.

As a Patient Care Physical Therapist, you will work collaboratively with other healthcare professionals to deliver high-quality patient care. We offer a competitive salary range of $40.32 - $58.46 per hour, as well as comprehensive benefits including medical/dental/vision coverage, health savings accounts, life insurance, mental health counseling, and opportunities for advancement.
