At Team Select Home Care, we're committed to delivering exceptional care to our patients in the comfort of their own homes. As a Licensed Nursing Assistant, you'll play a vital role in our mission to provide compassionate and personalized care.
Responsibilities- Provide skilled personal care and activities of daily living (ADLs) to patients under the direction and supervision of a registered nurse (RN).
- Perform and assist with direct patient/client personal care, household chores, meals, and other assigned/delegated tasks.
- Assist with simple procedures as an extension of therapy or nursing services as assigned/delegated.
- Maintain excellent infection control practices at all times, especially handwashing and appropriate precautions as directed by the RN.
- Understand and adhere to established Agency policies and procedures.
- Complete visit records in a timely manner as per Agency policy.
- Report changes in the patient's condition timely as well as other needs to the RN.
- Perform household services essential to healthcare in the home as assigned.
- Attend staff meetings and patient care conferences as scheduled.
- Follow Agency policy for cleaning equipment between patient uses.
- Carry out, report, and document care given in an effective, timely manner.
- Complete all work assigned by the RN; report incomplete assignments to the RN.
- Do not accept assignments for a patient for which he/she has not received appropriate training.
- When applicable under state law, follow and perform RN delegation tasks as assigned.
- Observe confidentiality and safeguards for all patient-related information.
- Report any complaint, infection, variance, accident, or unusual occurrence to the RN.
- Participate in QAPI activities as requested/assigned.
- Maintain acceptable attendance status, per Agency policy.
- Appearance is always within Agency standard; is clean and well-groomed.
- Demonstrate effective time management skills through daily documentation and infrequent overtime for routine assignments.
- Attend position-related in-services as assigned. Attend all mandatory in-service programs as scheduled; minimally 12 hours/year.
- Maintain clean and neat work environment.
- Demonstrate sound judgment and decision-making.
- Maintain current CPR certification and health certifications per state requirements.
- Maintain all human resource renewables as directed.
- Perform other duties as assigned/delegated.
- High School Diploma or GED.
- 18 years of age or per state regulation.
- Maintain licensure or certification per state regulation.
