At So IL Health Care Foundation, we are seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare organization.
Job Summary:The Registered Nurse will be responsible for planning and implementing individualized nursing care based on accurate assessments and evaluations. You will work closely with our interdisciplinary treatment team to provide comprehensive care to our patients.
Key Responsibilities:- Manage patient flow and prepare patients for examinations by providing clear explanations and assisting providers during patient exams.
- Assess vital signs, reason for visit, signs/symptoms reported by patients, patient history, social history, patient allergies, current medications, laboratory/radiologic results, and patient's level of understanding and/or need for accommodation and support.
- Establish personal rapport with patients and other individuals to understand care requirements.
- Develop and implement educational plans using patient diagnosis, quality measures, health stage, age-related recommendations, and available teaching materials.
- Check for and/or obtain signed informed consent forms for required procedures and Release of Information from patients.
- Assist with routine examinations and procedures, provide simple explanations, and encourage patients as necessary.
- Assess and refer patients for support services necessary for improved patient health outcomes.
- Check for the presence of allergies, administer medications as ordered by providers, and observe patients for allergic reactions.
- Assess patient health status over the telephone, determine a plan of action, or advise patients on self-care/monitoring.
- Communicate clearly and effectively with patients, team members, providers, and patient family members.
- Authorize medication refills from verbal, written, or provider protocol orders, with appropriate documentation of refill and ensuring follow-up.
- Provide support and assistance in other departments/sites as needed or directed.
- Assist with maintaining all required tracking within the health center system.
- Function within the legal and ethical parameters of nursing care, as a member of the interdisciplinary treatment team and according to the policies of So IL Health Care Foundation.
- Educate staff members on current knowledge acquired through continuing educational opportunities.
- Identify problem areas with supervisors/health center managers, implement a plan of action to bring about change, and resolve patient problems and needs.
- Assist supervisors/health center managers in seeking and monitoring quality improvement and clinical performance metrics related to patient care.
- Recommend process improvements/changes to increase compliance and efficient nursing time management.
- Know infection control procedures and other safety and security protocols to keep patients and staff safe.
- The Registered Nurse may be responsible for training new hires/students and assessing the clinical competencies of MA's, LPN's, and other RN's.
- The Registered Nurse will work their assigned clinical inbox daily and remain within the assigned threshold parameters.
- Maintain and track all supplies and equipment as assigned.
- Perform other duties as assigned.

Requires a degree from an accredited School of Nursing and a current license to practice as a Registered Nurse in the State of Illinois. Must have and maintain BLS certification.

Understand role, complete relevant duties, and the requirement to maintain the highest level of proficiency within scope, as part of this team-based approach in the patient care continuum.
Physical Demands:Must be able to lift up to 50 pounds. Must be able to assist in patient lifts/transfers utilizing proper body mechanics. Must be able to stand for extended periods and walk on a regular basis.
Work Schedule:Normal work hours for this position are Monday through Friday dayshift unless otherwise specified.
