Key Responsibilities- Admission and Re-Certification: Utilize Oasis to admit, transfer, re-certify, and discharge home health patients in accordance with physician orders.
- Patient Care Coordination: Coordinate and supervise LPNs and HHAs in the delivery of patient care, ensuring compliance with agency policy and procedures.
- Medicare and Medicaid Compliance: Maintain adherence to Medicare and Medicaid regulations, ensuring accurate documentation and reporting.
- Patient Education: Educate patients and caregivers on disease processes, medications, and individualized treatment plans.
- Supervision and Training: Supervise and train LPNs and HHAs, ensuring they possess the necessary skills and knowledge to provide quality patient care.
- Communication and Collaboration: Communicate effectively with physicians, clinical managers, therapists, and case managers to ensure seamless patient care.
- Qualifications: Be a registered nurse (R.N.) with a current license and a minimum of one year of nursing experience as a R.N.
- Work Environment and Physical Requirements: Work in a fast-paced office environment with moderate noise levels, controlled temperature conditions, and frequent travel to patient homes.
