POSITION OVERVIEW:
To be fully committed to delivering Quality/No Harm, Customer Experience, and Stewardship: by ensuring the quality, timeliness, and effectiveness of stroke care delivery for Health First;


by providing supervision and monitoring of a patient when a Form 52: Baker Act Declaration, Suicide Precautions are necessary, or whenever it is assessed that a patient's safety may be at risk without supervision.


KEY RESPONSIBILITIES:

  • Associates uphold engagement through adherence to the (I-CARE) principles.
  • Promptly communicates clear, concise, and relevant information to the charge nurse or nurse assigned to the patient regarding the patient's condition.
  • Immediately notifies nursing staff of any potential safety concerns.
  • Maintains vigilance throughout the entire shift.
  • Never leaves the patient unattended unless authorized by the patient's nurse (from memorable customer experience).
  • Employs clear and effective communication skills in professional interactions with all healthcare associates, patients, and families.
  • Reports to the charge nurse or nurse assigned to the patient at the start of each shift for expectations regarding specific behavioral and restraint instructions.
  • Provides a shift summary to the charge nurse or nurse assigned to the patient at the end of each shift.
  • Immediately communicates clear, concise, and relevant information to the charge nurse or nurse assigned to the patient regarding the patient's condition.
  • Immediately alerts nursing staff of any potential safety concerns.
  • Remains vigilant throughout the entire shift.
  • Maintains a professional, neutral stance in all discussions with the patient and/or family.
  • Never leaves the patient unattended unless authorized by the patient's nurse.
  • Respects the patient's privacy and refrains from using the patient's telephone or personal belongings.
  • Contributes to the patient experience as evidenced by positive patient satisfaction scores and HCAHPs scores for the department/medical center, as well as written compliments from patients and others, along with consistent performance in addressing patient needs hourly (i.e.: positioning, proximity, personal needs [restroom], pain/comfort).

ADDITIONAL REQUIREMENTS:

  • High School diploma or equivalent.
  • Must be able to comprehend and communicate written and verbal instructions in the English language.

  • Must possess reliable transportation to travel among all Health First facilities.
