Key Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is tasked with assessing and evaluating the care needs of patients concerning their functional status, mobility, endurance, pain levels, and other medical conditions. Following this assessment, the Physical Therapist collaborates to create a tailored treatment plan, implements interventions to improve the patient's health, and monitors their progress.

  • Assess the patient's functional capabilities and physical therapy requirements, assisting in the development of a comprehensive therapy plan.
  • Monitor and report the patient's responses to treatment and any changes in their condition to the supervising nurse or physician.
  • Provide treatments aimed at alleviating pain, restoring function, and maintaining optimal physical capabilities in accordance with established standards of practice.
  • Educate and inform the healthcare team, patients, and their families or caregivers about the care plan, functional limitations, and progress towards established goals.

Qualifications

Licensure Requirements

  • Valid Physical Therapy license in the relevant state
  • Current CPR certification
  • Possession of a valid driver's license, vehicle insurance, and access to a reliable vehicle
