Home Health LPN Job Description

As a Home Health LPN with Interim HealthCare, you'll have the flexibility to create your own schedule and work-life balance. Our home care company is looking for passionate and dedicated LPNs to provide exceptional patient care in the comfort of their own homes.

Job Responsibilities:

  • Provide home-based care to patients with various health conditions under the supervision of an RN
  • Collaborate with a multidisciplinary team to deliver quality patient care
  • Take patient vitals, conduct assessments, and document progress
  • Assist RNs in implementing patient care plans and teaching self-care techniques
  • Educate patients and families on medication management and home safety
  • Participate in interdisciplinary team meetings to coordinate patient care and plan for discharge

Requirements:

  • Graduate of an accredited practical nursing program and active LPN license in PA
  • Minimum of 1 year of LPN experience, preferably in home healthcare or completion of our preceptor program
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
