Job Summary

The Lead Community Health Worker serves as a role model to the team by promoting a positive and professional demeanor towards neighbors, staff members, and team. This position supports daily operations of the CHW team and a small case load of neighbors and clients. The Lead CHW will work closely with the program manager to ensure high-quality service for all neighbors and patients served by the CHW program.

Key Responsibilities
  • Support daily operations of the CHW team
  • Manage a small case load of neighbors and clients
  • Collaborate with the program manager to ensure high-quality service
  • Develop and implement community outreach and education programs
  • Provide training and support to CHW team members
Requirements

To be successful in this role, you will need:

  • Bachelor's Degree or equivalent education/experience
  • 5 years' experience in a Community Health Worker or Community Health Worker-like role or commensurate lived experience
  • 2 years of experience related to community outreach, enrollment, and education
  • 1 year of experience supporting teams or leading projects
