Breast Cancer Patient Navigator/ BCN

Under the leadership of Oncology, the Cancer Patient Navigator serves as a vital member of a multidisciplinary team, providing advocacy and education to cancer patients from diagnosis through end of active treatment, and possibly end of life.

The Cancer Patient Navigator offers critical assistance to patients, families, and caregivers to overcome system barriers and facilitate timely access to quality care.

Patient navigation involves assessing the physical, psychological, and social needs of patients.

Key Responsibilities:
  • Facilitates appointments for patients with diagnostic mammograms, educating on additional testing or procedures necessary, including ultrasound or biopsy at the point of a positive result for Breast Cancer.
  • Facilitates referrals to Pulmonology for patients with positive lung nodules.
  • Assists patients in understanding their diagnosis, treatment options, and available resources, including clinical research studies and facilitating surgical or medical oncology referrals as ordered by the physician.
  • In conjunction with the Genetics Navigator, ensures all patients are screened, educated, and undergo genetic testing.
  • Post-Op, ensures timely follow-up with medical oncology.
  • Serves as a link between patients and other care providers, coordinating multidisciplinary case conferences and educating staff in Radiology and Oncology departments.

  • Registered Nurse license (RN)
  • Bachelor of Science in Nursing (BSN)
  • Maintain CPR certification
  • Oncology nursing experience preferred
  • Radiology experience beneficial
