Job Summary:
We are seeking a skilled Occupational Therapist to join our team in Ripon, WI and surrounding areas. As a Geriatric Rehabilitation Therapist, you will work closely with our interdisciplinary team to provide comprehensive care to older adults, focusing on functional outcomes and patient-centered care.
Responsibilities:
- Provide occupational therapy services to older adults in their communities, focusing on functional outcomes and patient-centered care.
- Work collaboratively with our interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Implement evidence-based interventions to promote functional independence and quality of life.
- Document patient progress and outcomes in a timely and accurate manner.
Requirements:
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to apply.
- Degree from an accredited occupational therapy program.
- Basic computer literacy skills.
- Current CPR certification.
