Home Health Registered Nurse Job Description

VNA Health Group is seeking a skilled and compassionate Registered Nurse to provide high-quality home health care to patients in Essex County, New Jersey.

Responsibilities:
  • Assess and evaluate patient needs, developing individualized care plans to promote optimal health outcomes.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and families.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Monitor patient progress, updating care plans and outcomes as needed.
  • Identify teaching needs and develop strategies to educate patients and families on health management and self-care.
Requirements:
  • RN with Bachelor's degree in Nursing preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills and ability to navigate electronic medical records.
  • Excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
