About the Role

We are seeking a skilled Physical Therapist to join our team at Commonwealth Home Health - Winchester, a part of LHC Group. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

Key Responsibilities
  • Evaluate patient's functional status and physical therapy needs
  • Assist physician in developing therapy plan of care
  • Observe and report patient's response to treatment and any changes in condition to supervising nurse and/or physician
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
  • Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals
Requirements
  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
