Job Summary:
We are seeking a compassionate Registered Nurse (RN) to join our team as a Nurse Care Coordinator. This role involves working closely with patients and their families to develop and implement individualized care plans, as well as providing education and support to ensure seamless care transitions.
Key Responsibilities:
- Conduct thorough assessments of patients' medical and social needs
- Develop and implement comprehensive care plans that address these needs
- Collaborate with multidisciplinary teams to ensure timely and effective care delivery
- Provide education and support to patients and families on disease management, medication adherence, and other relevant topics
Qualifications:
- Current RN license in good standing
- 6 months prior hands-on nursing experience
- CPR certification
- TB skin test within last 12 months
- Reliable transportation
