As a RN Case Manager at Visiting Nurse and Hospice for VT & NH, you will play a vital role in delivering exceptional home health and hospice services to our patients. We are seeking a skilled and compassionate Registered Nurse to join our team.
About the Role:As a RN Case Manager, you will be responsible for:
- Developing and implementing patient care plans in collaboration with the patient's physician and interdisciplinary team members.
- Conducting regular assessments of the patient's clinical, functional, and emotional status to identify problems and progress towards desired goals.
- Coordinating patient care through effective communication and case conferences with the interdisciplinary team.
- Providing education and support to patients and caregivers on self-care techniques, medication management, and other health-related topics.
To be successful in this role, you must:
- Be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Have a graduate degree from an accredited school of nursing.
