About the Role

We are seeking a skilled Speech-Language Pathologist to join our team of healthcare professionals in providing high-quality home-based speech therapy services to patients in need.

Key Responsibilities
  • Provide individualized speech therapy sessions to patients in their homes, addressing communication, swallowing, and cognitive difficulties.
  • Collaborate with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, PTs, and OTs, to develop and implement patient care plans.
  • Conduct thorough assessments, document progress, and ensure patient goals are met.
  • Develop and implement effective treatment plans, including exercises and strategies to improve communication, swallowing, and cognitive function.
  • Educate patients and their families on therapy goals, safety practices, and self-care techniques.
Requirements
  • Master's or Doctoral degree in Speech-Language Pathology and active SLP license in PA.
  • Minimum of 1 year of speech therapy experience, preferably in home healthcare.
  • CPR certification.
  • Knowledge of state and federal home health regulations.
  • Excellent clinical judgment, critical thinking skills, strong communication, empathy, and compassion.
