We are seeking a skilled Registered Nurse to join our Adult Behavioral Health team at Carle Foundation Hospital. As a member of our interdisciplinary team, you will contribute to the provision and management of care to patients through the application of the nursing process.
Responsibilities- Assess patient's status through interview, observation, and physical assessment of actual problems and outcomes.
- Develop an individualized plan of care using a collaborative approach with the patient/family and healthcare team.
- Use appropriate interventions identified in the plan of care to achieve expected patient outcomes.
- Systematically evaluate and document patient's progress toward expected outcomes/goals on an ongoing basis throughout the shift.
- Modify plan of care based on evaluation.
- Complete required documentation as appropriate, including patient chart and event reports.
- College Diploma in Graduate of an accredited nursing program.
- Registered Professional Nurse (RN) License Illinois upon hire.
- Crisis Prevention Institute (CPI) certification within 3 months if hired after August 1, 2019.
