Support Practitioner

We are seeking a dedicated and compassionate Support Practitioner to join our Care at Home team in Seaboard. As a Support Practitioner, you will play a vital role in supporting vulnerable individuals in their own homes, promoting their independence and well-being.

• Provide person-centered support to individuals in their own homes, promoting their independence and dignity.
• Develop and maintain positive relationships with service users, their families, and other stakeholders.
• Work collaboratively with the Care at Home team to ensure seamless delivery of care services.
• Contribute to the development and implementation of individual care plans, ensuring they are tailored to meet the unique needs of each service user.
• Participate in ongoing training and professional development to enhance your skills and knowledge in supporting vulnerable adults.
