Job Overview

Role Summary: The After Hours Nurse is tasked with delivering direct patient care during non-business hours.

Qualifications:

  1. Completion of an accredited Nursing Program.
  2. Active LPN license in the State of Ohio.
  3. At least two years of experience in Medical/Surgical, Oncology, or community health nursing.
  4. Experience in hospice care is preferred.

Essential Skills and Requirements:

  1. Possession of a valid driver's license.
  2. Exceptional customer service abilities.
  3. Meticulous attention to detail with strong communication, documentation, and organizational skills. Capability to follow directives.
  4. Ability to work autonomously without supervision.
  5. Willingness to travel to various locations as necessary, including during adverse weather conditions.
  6. Capacity to lift supplies or equipment weighing up to 25 lbs.
  7. Physical demands include varying amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
  8. Ability to assist with patient care tasks such as ambulation, transfers, and positioning.

Key Responsibilities:

  1. Deliver proficient nursing care to assigned patients through effective symptom management, emotional support, education, and timely responses to patient and family needs, including:
    1. Routine home care visits
    2. End-of-life and continuous care
    3. Referrals
    4. Assistance at care centers
    5. Chart reviews
    6. Additional duties as assigned.
  2. Provide patient and family services in accordance with the established Plan of Care and signed physician orders.
  3. Promptly recognize and report any changes in the patient's mental and/or physical condition to the registered nurse, both in written and oral formats as appropriate.
  4. Clarify hospice services and benefits concerning Medicare, Medicaid, and private insurance, and understand when to implement palliative care interventions and advance beneficiary notices.
  5. Obtain and document orders from Medical Directors and Physicians as necessary.
  6. Order suitable Durable Medical Equipment (DME) for patients.
  7. Ensure continuity of care by coordinating the admission of home-care patients to inpatient settings (hospitals, nursing homes) as required.
  8. Work evenings and weekends as necessary to meet the needs of patients, families, and agency backup schedules.
  9. Complete all required documentation in compliance with organizational policies, accreditation standards, and regulatory requirements.
  10. Maintain clinical competency for assigned responsibilities and procedures.
  11. Perform other duties as assigned.
