Job Overview

The Registered Nurse Case Manager provides comprehensive care coordination and management for clients in Delaware, ensuring continuity and appropriateness of care while addressing various health and social needs. This role involves traveling to members' homes and nursing facilities to facilitate face-to-face interactions and subsequent telephonic contact.

Key Responsibilities:
  • Assess, plan, coordinate, implement, and evaluate care for eligible members with complex healthcare and social service needs.
  • Travel to members' homes, nursing facilities, and other community-based settings to complete assessments and provide care coordination.
  • Collaborate with healthcare teams to ensure continuity and appropriateness of care.
  • Facilitate transitions to alternate care settings and educate members or caregivers regarding healthcare needs, available benefits, resources, and services.


Requirements:
  • Licensed Registered Nurse (RN) in the state of Delaware.
  • 5+ years of case management experience, particularly with complex healthcare and social support needs.
  • Experience working with geriatric populations and individuals with behavioral health needs.
  • Ability to communicate effectively with members, families, and healthcare teams.


Salary:
$80,000 - $110,000 per year, depending on experience.
