Job Title: Occupational Therapist

We are seeking a highly skilled Occupational Therapist to join our team at Katherine Luther Residential. As an Occupational Therapist, you will play a vital role in helping our residents achieve their maximum potential and independence.

Job Summary:

The Occupational Therapist will be responsible for evaluating and treating residents who have been referred to occupational therapy by their physician. This will involve identifying residents who may benefit from therapy and requesting orders for evaluation and treatment.

Essential Job Functions:
  1. Reviews referrals for occupational therapy and physician's orders; performs evaluations, sets goals, devises treatment plans, and implements treatments based on diagnosis, resident condition, and rehabilitative prognosis.
  2. Makes recommendations, sends progress notes, and consults with the physician, as necessary.
  3. Maintains proper documentation in medical records, including evaluations, progress notes, discharge summaries, and treatment logs.
  4. Provides information for care planning, MDS, and quality assurance.
  5. Supervises occupational therapist assistants concerning the carrying out of treatment programs.
  6. Communicates with the interdisciplinary team regarding patient care and consults with families as needed.
  7. Acts as an occupational therapy consultant to all departments, providing in-services as necessary.
  8. Uses effective communication techniques within the rehabilitation department and inter-departmentally to maximize team building.
  9. Maintains 85% productivity.
Work Environment:

The Occupational Therapist will work in a clinical rehabilitation department setting and in patient rooms, where contact with staff, physicians, residents' families, and the public is required.

Requires walking up to 95% of the workday. Must be capable of pushing, pulling, and moving up to 50 pounds or more. Finger dexterity is required with eye-hand coordination. Vision and hearing corrected to normal range.

Accountabilities:
  1. Reports to the Director of Rehabilitation.
  2. Treats patient information as confidential materials.
  3. Maintains the safety of patients and self during treatment.
  4. Accurate and timely record keeping.
  5. Follows nursing home policies and procedures.
  6. Respects residents' rights and dignity.
  7. Reports any suspected resident abuse as required by government regulations and the nursing home policies.
  8. Attends all required in-services and seminars.
  9. Mandatory in-services must be met within the required time.
  10. Required to follow established policies regarding tardiness, attendance, and dress code.
  11. Serves as a positive ambassador to Community Wellness Partners, their affiliates, residents, staff, visitors, and the community.
Qualifications:

Work requires the knowledge of occupational therapy normally acquired through an accredited University for occupational therapy. New York State licensure to practice occupational therapy is required.

Must possess skills to effectively acquire pertinent information from patients' charts and the patients to evaluate, implement procedures, and document the effects of treatment.

Interpersonal skills necessary to communicate accurately and precisely to patients and family members any instructions, which may be necessary in the completion of a treatment program.

Must be able to identify changes in patient conditions and be able to communicate those changes to the appropriate staff member.
