About the Role

We are seeking a highly skilled and detail-oriented Referral Coordinator to join our team at CenterWell Home Health. As a Referral Coordinator, you will play a critical role in preparing and processing managed care referral paperwork, scheduling appointments, and pre-registering patients for exams and procedures.

Key Responsibilities
  • Prepare and process all managed care referral paperwork in a timely and accurate manner.
  • Schedule appointments and pre-register patients for exams and procedures with specialists and providers outside of the primary care physician's office.
  • Communicate effectively with physicians, nurse practitioners, and nursing staff to gather pertinent information for referrals.
  • Maintain accurate and up-to-date referral records and files.
  • Respond to patient inquiries regarding health plan referrals in a professional and courteous manner.
  • Maintain confidentiality and adhere to all HIPAA guidelines and regulations.
Requirements
  • 1-3 years of experience in a clinical setting, preferably within the Medicare population.
  • Proficient in medical terminology and Microsoft Office applications, including Word, Excel, and Outlook.
  • Experience with Electronic Medical Records (EMRs) and ability to learn new systems quickly.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, physicians, and other healthcare professionals.
  • Bilingual in English and Spanish, with the ability to speak, read, and write in both languages without limitations or assistance.
Preferred Qualifications
  • Bachelor's Degree in Healthcare or Business Administration.
  • Knowledge of ICD-10 and CPT coding.
  • Medical Assistant or Professional State Certifications related to healthcare.
