Case Manager

Mental Health Resource Center is seeking a highly skilled and compassionate Co-Responder Case Manager to join our team. As a Co-Responder Case Manager, you will work closely with individuals in need of services who have been identified by our Mental Health Professionals and the Jacksonville Sheriff's Office.
Job Summary:The Co-Responder Case Manager will assess the individual's needs and coordinate a plan of care and/or treatment plan. You will conduct outreach and help establish aftercare plans for identified individuals in inpatient psychiatric facilities or jail. Your role will also involve facilitating linkage with resources and completion of established care plans.
Responsibilities:- Deliver case management services to individuals in need of ongoing support and services.
- Conduct initial and ongoing assessments to identify individual needs and develop a care plan.
- Collaborate with individuals, families, service providers, and significant others to develop a comprehensive care plan.
- Complete comprehensive risk assessments and biopsychosocial assessments on eligible participants.
- Monitor individuals to ensure effective community services and provide outreach at the jail to assist inmates with developing a plan for care upon release.
- Provide follow-up within 24 hours when an individual is discharged or released from an inpatient facility or jail.
- Document or request documentation of past medical, psychiatric, substance abuse, and social history for each individual.
- Ensure care plans and/or treatment plans contain all required services and signatures.
- Maintain accurate and comprehensive information in each record.
- Compile and submit reports as assigned.
- Serve as an advocate, ensuring rights as delineated in State and Federal Regulations.
- Bachelor's degree in Social Work or a related Human Services field from an accredited university or college.
- One year of experience working in human services or a mental health-related field.
- Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System.
- Strong communication skills and ability to interact with internal and external customers, families, caregivers, community service providers, supervisory staff, and other department professionals.
This is a full-time days position with Mental Health Resource Center, a not-for-profit Florida corporation providing mental health and behavioral health care services to the community.
